    SUBACUTE UNIT

    The 28-bed subacute unit at Park View Hospital includes a specialized neurorehabilitation program, which provides services to individuals with acquired brain injuries, neuropsychiatric disorders and other neurological/behavioral disorders.

    Our program also includes:

    • development of skills for community re-entry
    • vocational and pre-vocational opportunities
    • recreational programs and volunteer opportunities

    Upon admission to the program, an interdisciplinary team carefully creates an individualized care plan for each patient. Input is gathered from patients and their families as goals are created and the focus is on helping the patient return to the highest level of function.

    Within each patient’s care plan, objectives will be developed to accommodate his or her individual strengths and challenges. These address the status of communication, physical, medical, behavioral, social and other skills. Priority is given to the skills needed in the projected discharge setting and positive reinforcement is used as patients accomplish their goals.

    Deficiency-Free Survey, 2011

    Kindred Hospital Park View Subacute Unit received a deficiency-free survey from the Massachusetts Department of Health in 2011. 

    Each state conducts onsite inspections that determine whether its subacute units meet the minimum Medicare and Medicaid quality and performance standards. The State conducts inspections of each subacute unit that participates in Medicare and/or Medicaid about once a year. During the subacute inspection, the State looks at many aspects of quality. The inspection team consists of trained inspectors, including at least on registered nurse.

    Five-Star Rating, September 2011

    5 Star Rating

    Kindred Hospital Park View Subacute Unit has been honored with a Five-Star rating in September 2011 from the Centers for Medicare and Medicaid Services (CMS). CMS created the Five-Star Quality Rating System to help consumers, their families, and caregivers compare subacute units more easily and to help identify areas about which you may want to ask questions. The quality rating system gives each nursing home a rating of between 1 and 5 stars. Subacute units with 4 or 5 stars are considered to have “above average” quality.
